Please call our office 24 hours in advance if you are unable to make your appointment or would like to reschedule. Unless an appointment is cancelled 24 hours in advance, our policy is to charge $25 after the second missed appointment.

Note: If you are more than 15 minutes late, we will notify your provider to see if they can still see you. If not, you will need to reschedule your appointment.

Please bring your Insurance card at each visit.
